Release to Parent from Athletic Event →
This form is ONLY required on the occasion(s) your son cannot travel via bus with the team to and/or from games.
Field Trip by Private Vehicle →
Parents MUST return the FIELD TRIP FORM with a copy of their CDL and proof of insurance if they plan to drive their own athlete to and from a game/tournament – and if they might volunteer to drive others. Please let your team parent know if you can drive carpool – the Field Trip form applies to that too.
Student Athletes that drive MUST ALSO submit a form to drive themselves to and from games and tournaments. They cannot drive other students to or from games.
